What is the Community Learning Disability Team?

The Community Learning Disability Team (CLDT) supports adults with a learning disability to stay healthy, safe and well in their community

The team works with the person, their family, carers, and other services.

Registered nurses and health specialists

The Community Learning Disability Team has two main teams that work closely together.

  1. The Health Team

Support with health needs like epilepsy, physio and speech and language

Learning Disability Social Workers. Work for the Local Authority

Support with care, daily life and staying safe

What do the Community Learning Disability Team do?

Listens to the person

Gives the person time to understand and makes reasonable adjustments

Works with other services to make sure people get fair and equal care.
